Cleveland has won five straight and shows up at Fenway with real division-title stakes, but Boston still controls its own playoff fate against a Guardians lineup that ranks near the bottom of MLB in scoring.
Quick take:
- Pick: Red Sox -1.5
- Why: Boston’s bullpen depth and home-park edge overcome a shakier recent stretch for the Sox rotation
- The number to watch: Parker Messick’s 2.57 ERA is the best of any starter in this series, but Cleveland’s offense is averaging under four runs a game on the road this month
The Guardians (81-75) arrive in Boston as the AL Central’s first-place club, riding a five-game winning streak and a 7-3 mark over their last 10 that’s carried them a full game ahead of the White Sox. Foster Griffin (15-5, 3.67 ERA) gets the ball for Cleveland, leaning on a rotation that’s quietly been one of the better stories in the league this month.
Boston (84-72) still sits comfortably inside a playoff spot, holding a 4.0-game cushion in the wild-card race, but the Red Sox have dropped four of their last six and were just outscored in a series loss at Tampa Bay. Payton Tolle (9-6, 3.02 ERA) starts for Boston, and left-hander vs. left-hander is the theme of this opener — both starters have limited opponents to sub-.220 batting averages this season.
Boston opened as a -130 favorite on the moneyline with the Guardians sitting at +110, and the run line has settled with the Red Sox at -1.5. That number has bounced around slightly between MLB odds books — some had Cleveland’s runline price as tight as -205, others closer to -220 — but the shape of the market hasn’t changed: Boston’s home-field edge and Tolle’s strong strikeout rate (10.45 K/9) are keeping the Red Sox as modest favorites even against a team playing its best baseball of September. The total sits at 6.5, and with two pitchers running sub-.220 opponent averages, the under has drawn plenty of the sharper action.
| Tuesday, September 22 at 6:46 PM | Spread | Money | Total |
| Cleveland Guardians | +1.5 (-220) | +110 | O 7 (-108) |
| Boston Red Sox | -1.5 (165) | -127 | U 6.5 (104) |
Cleveland’s five-game streak tells a different story than Boston’s last two weeks — the Guardians are scoring and preventing runs at a level that simply hasn’t matched their season-long numbers, while the Red Sox have been treading water since mid-September.

Boston’s relief corps has been the best in baseball by ERA this season, and that matters against a Guardians lineup that ranks 28th in MLB in OPS. Cleveland’s pitching has been the story of their streak, but their own bats have to solve a bullpen fronted by Garrett Whitlock (1.78 ERA) and Jovani Morán. Trevor Story has been Boston’s igniter of late, riding an 11-game hitting streak and batting .338 since returning from injury in late August — his presence at shortstop gives the Red Sox lineup a jolt right when they need it.
Cleveland counters with a bullpen of its own that’s been arguably even better in shutting down free-swinging lineups — Cade Smith and Hunter Gaddis have combined to give the Guardians a real late-inning identity. But the Guardians have to get through six or seven innings first, and their .692 team OPS on the road makes stringing together rallies against a deep Boston pen a tougher ask.
The Guardians are averaging just 4.2 runs allowed and 5.6 scored over their last 10 games, a differential that flatters a stretch built largely against sub-.500 competition in the White Sox and Athletics — Boston is a sharper test.
That context matters. Cleveland’s hot streak came against the White Sox and Athletics, two teams well outside playoff contention. Facing a Red Sox club that still has something to play for, at Fenway, is a different environment than padding a division lead against also-rans.
Boston’s own injury list is worth watching — Willson Contreras (hand), Ceddanne Rafaela (quadriceps), and relievers Aroldis Chapman and Garrett Crochet are all dealing with something, which thins the pool of late-inning options and puts even more staff on Whitlock and Morán’s arms.
Cleveland’s rotation and bullpen are legitimately good enough to make this close, and Messick’s 2.57 ERA gives them a puncher’s chance in a low-scoring game. But Boston’s lineup depth, its bullpen edge, and Fenway all tip this one toward the home club.
The pick:
Take Boston to get back on track behind Tolle and a bullpen that should keep this Cleveland offense in check for most of the night.
